You can register as a patient of the practice provided that you are resident within the practice area.
To register, you will need to attend at the surgery in order to complete the necessary paperwork and to provide 2 forms of identification.
The forms required can be downloaded from the links below for you to complete in advance if you prefer. YOU MUST COMPLETE both forms (GMS1 & patient questionnaire) in full as part of the registration process.

Identification
To comply with recommendations from the NHS Counter Fraud Service we ask new patients to provide us with 2 forms of identification, one from each of the lists below. If you are unable to meet with these requirements then please discuss this with us in the first instance.
List1: Passport/Photographic Driving Licence/Birth Certificate/Marriage Certificate/Medical Card
List2: Paid Utility Bill/Bank or Building Society Statements/Local Authority Rent Card/Council Tax Bill/Letter from Benefits Agency/Benefits Book/Signing on Card/Papers from Home Office/P45/P60/Payslip
New Patient Health Check
We encourage all new patients to book a new patient health check. This takes approximately 20 minutes to complete, and,whilst not compulsory,it enables us to make sure that our information is as up to date as possible from the start of your time as a patient of the practice.
